Origin and Cause
A Fire 14 Months in the Making
A small fire started within a drying oven at a floor cleaning pad manufacturing plant. For what had, as of late, become a familiar occurrence within drying ovens at the plant, the fire was extinguished using a nearby handheld fire extinguisher. With only minor damage, the manufacturing process resumed. Shortly after the drying oven fire was extinguished, a substantial fire developed within the wet scrubber system, requiring the involvement of a municipal fire department. The wet scrubber system had been installed just 14 months prior to the fire in order to comply with a state mandated air quality permit, was destroyed by the fire. Following the fire, both the wet scrubber system and the floor cleaning pad manufacturing process were inspected. The history of the air quality permit associated with the wet scrubber system and its stipulated hazardous air pollutants (HAPs) emissions were researched. Specifications regarding the design of the wet scrubber system and the engineering drawings affecting its installation were examined.
